Derby 1-0 West Brom (2007)
In the first Championship play-off final to be played at the rebuilt Wembley, Billy Davies’ Derby booked their place in the Premier League with a tense 1-0 victory.
West Brom dominated for long spells with Diomansy Kamara, Jason Koumas and Paul Robinson all forcing Stephen Bywater into saves, while Kevin Phillips also went close, hitting the crossbar.
But it was Derby who eventually came out on top as Stephen Pearson converted Giles Barnes’ low cross to score the only goal of the game to seal promotion.
Bristol City 0-1 Hull (2008)
A Dean Windass volley fired Hull into the Premier League for the first time in their history as they beat Bristol City at Wembley.
A cagey affair was played out in front of almost 87,000 but one moment of quality settled the encounter, hometown hero Windass smashing home from Fraizer Campbell’s pass.
Blackpool 3-2 Cardiff (2010)
Ian Holloway’s Blackpool twice came from behind to defeat Cardiff and reach the Premier League for the first time after an entertaining encounter where all the goals came in the first half.
Michael Chopra and Joe Ledley twice put Cardiff ahead but a sublime Charlie Adam free-kick and further goals from Gary Taylor-Fletcher and Brett Ormerod turned the tie on its head.
The result capped a sensational first season in charge for Holloway, whose Blackpool team went on to light up the Premier League but suffered a heartbreaking final-day relegation back to the Championship.
Reading 2-4 Swansea (2011)
Swansea City became the first Welsh side in history to reach the Premier League after beating Reading 4-2 at Wembley in the play-off final.
Brendan Rodgers earned redemption against his old club after seeing his side clinch a thrilling encounter that swung one way and then the other.
QPR 1-0 Derby (2014)
QPR clinched promotion to the Premier League in the most dramatic style as they beat Derby 1-0 in the Championship play-off final thanks to Bobby Zamora’s 90th-minute winner, despite being reduced to 10 men after an hour.
On a tense afternoon at Wembley, QPR had to withstand a Derby onslaught in the second half after Gary O’Neil’s straight red card for a foul on Johnny Russell.
But Zamora, who hit a play-off winner for West Ham in 2005, pounced in the final minute to dash Derby’s dreams and secure a return to the top flight for QPR just a year after being relegated.
Middlesbrough 0-2 Norwich (2015)
Norwich City sealed an immediate return to the Premier League as a blistering first-half display saw them beat Middlesbrough at Wembley.
Two goals in three first-half minutes shifted the final in favour of Alex Neil’s side as goals from Cameron Jerome and Nathan Redmond saw the Canaries return to the top flight at the first time of asking.
Hull 1-0 Sheffield Wednesday (2016)
Mo Diame struck a sensational winning goal 18 minutes from time as Hull secured promotion back to the Premier League after beating Sheffield Wednesday 1-0 in the Sky Bet Championship play-off final.
It needed something special to beat Wednesday goalkeeper Keiren Westwood, who was in inspired form in front of 40,000 raucous Owls fans.
