He starts by noting that Cooper “has aged very gracefully.” That phrase stuck with me — it’s not a flat denial, and it’s not a full admission of makeover mania. It’s measured.

Mohan points to a few things that might explain the new look. First, the midface — the cheeks and the under-eye area — appears fuller and smoother than before. That could come from hyaluronic acid fillers, or from collagen-stimulating injectables like Sculptra or Radiesse. Fillers can soften shadows under the eyes and restore a roundness that time tends to flatten. You don’t need a knife for that; an injector can do quite a bit in a single session.

Second, the skin itself looks tauter and more even. That can be accomplished with energy-based treatments — think Ultherapy or radiofrequency, which tighten skin gradually — or with laser resurfacing, which refines texture. These aren’t surgical procedures either. They’re more like maintenance. They don’t rewrite bone structure; they just tidy up what’s already there.

Finally, the forehead. It doesn’t read as stiff, but it also lacks the little lines you’d expect from decades of expression. In other words, there could be a touch of Botox. It’s subtle, if Dr. Mohan is right. Botox smooths motion lines, and a small amount can make a face appear rested without freezing expressions completely.

What he doesn’t think happened — and that matters

Probably the most important part of Dr. Mohan’s assessment is what he doesn’t see. He doesn’t see altered facial harmony, changed bone structure, or frozen expressions — all the hallmarks of a substantial surgical facelift. That’s a different category of change. His read is that if Cooper has had anything dramatic, it’s not obvious. Instead, any work has likely been “strategic and very subtle.” In plain terms: some injectables, maybe skin tightening, possibly a little neurotoxin. Small moves, not a full overhaul.

A few thoughts — not definitive, but human

I find that plausible. For one, celebrities do invest in upkeep; it’s part of the job. Being on camera means minor imperfections look bigger. Also, modern non-surgical treatments have come a long way. Injectors can add targeted volume, lasers can smooth skin texture, and energy devices can lift without incisions. So even without a lot of drama, a lot can change over a few years.

On the other hand, lighting, styling, and the camera angle also matter a lot. A makeup job that hits the cheekbones just so, hair that frames the face differently, or a better skincare routine — all that adds up. People want a single, satisfying explanation (“he had a facelift”), but real life is usually layered: a few small procedures here, different grooming there, and time itself plays a role.

What bothers me a little is how certain some online voices get. I understand wanting to identify cause and effect, but the internet often leaps from observation to a definitive verdict without the nuance. Dr. Mohan’s assessment provides some of that nuance: he sees signs of cosmetic maintenance but not a complete surgical transformation. That’s not as dramatic, and maybe it’s less fun for gossip, but it’s probably closer to the truth.

Bradley Cooper’s face has changed a bit over the past decade — fuller midface, smoother skin, fewer lines.
